Overview: The asthma management program is designed to offer asthma education to your patient (including family members or caretakers). The program offers cost avoidance of ER visits and unnecessary hospitalizations. Our RN nurse coordinator is available for triage and support to your asthma patients. We offer education on all medications and ensure correct use of the breathing aids. Our program tracks medications (filled in our pharmacy) and our pharmacists will monitor the monthly prescriptions to be sure that medications are delivered on time.
Action Plan: Three skilled nurse visits that include complete environmental assessment, asthma education and color-coding of medications, with the option for PRN visits for additional education or during exacerbations. Your patient learns "when to seek medical attention". Our nurse educates the patient on proper use and care of equipment and devices. You, as the physician, have the option of an individualized asthma action plan for each patient or "standing orders" for all your asthma patients. Your patient may be enrolled for the initial three visits, a specific time span or for life.
- An easy form serves as the physicians order. This form enables you to order nebulizers, peak flow meters, spacers, spirometry, flutter valves (mucous clearing devices) and sleep studies (with single or dual Ph sensors).
- The asthma action plan enables the physician to set parameters for clear and concise patient management.
- The physician order form includes a section for your newly prescribed medications such as Epi-Pen©, albuterol, Xonenex©, Pulmicort©, etc. The form may be faxed directly to our on site pharmacy to save you time when calling in multiple medications.
The Preferred Asthma Management Program may be ordered along with the Pari Trek nebulizer or it can be initiated alone. We are happy to accommodate you with the plan that you will both oversee your patient and at the the same time reduce the demands on you and your staff.
